Examples of corruption in hospitals around GCC

Can you brief us about your experience?

I have lived and worked in the MENA-GCC regions since 1996 as a radiology engineer.


What is the difference between Radiology Engineering and Clinical Engineering?

Clinical Engineering generally encompasses Biomed and some facilities maintenance. Radiology Engineers do only Imaging systems.

Can you tell us about your experience with corruption at health facilities in GCC?

There have been numerous instances of corruption that I have witnessed. It has been sometimes a suggestion for a gift or as blatant as flatly being told an amount or percentage of the project that I would be required to pay if we were awarded the contract.

My first instance came as my company was servicing 2 CT scanners at a very large MOH facility, the local agent paid off the in-house Biomed engineer to complain that he was unhappy with our service. This is while one scanner was at 97% uptime and the other at 99%. As a result, we were not allowed to bid when it came contract renewal time.

Did you; recently, face any corruption incident? for example, were you asked to pay any amount of money to finalize some customer's project? 

My most recent situation was a couple of weeks back. I was contacted by another company about a project at a new private hospital. We were asked to bid for their DR, CT and MR systems. These were all going to be late model used units and we had them relocated. Our pricing was the low bid based on or cost and one year warranty cost. Then, the person that referred us wanted a 5% finder’s fee and the Consultant engineer wanted a 5% payback to him personally. These fees all came up AFTER we were low bid and had really cut our margins to be very competitive.

Is "corruption" a culture in health technology practice in  in GCC ?

Over the course or almost 17 years I have seen dozens of cases of people wanting payments or gifts and they feel that it is completely acceptable to do business that way.

In your point of view, what is the solution for corruption in healthcare technology industry in GCC?

Solving the issue of corruption is going to be a very difficult matter to address. Culturally it has been accepted for generations. Bakshish(Tip) has been a part of the Gulf society and a normal trading practice. However, with a matter as sensitive and important as healthcare, it is a mandatory change. Every time someone accepts something with a payment they are diminishing the quality of the service that is provided. Additionally, in the case of employees accepting bribes, they are stealing from the facility owner. If it Ministry facility, they are stealing from the government.

The solution is twofold.

  1. Persons that report it (whistleblowers) should be rewarded and recognized for their honesty. Not punished and made to seem as if they are at fault. 
  2. The other change is to heavily punish those caught accepting bribes. They should be dismissed from their position and have to make full restitution for the amount they took. If it is a government facility they should also face the risk of a prison sentence for stealing from the government.

Some Health IT Supply Management Concerns in Middle-East


Healthcare technology supply concerns in middle East are numerous. Some are normal and natural, others are not common. Here, I will only focus on two examples from the EMR market in GCC.
Natural Evolution.An example of SCM issues which are encountered as part of natural evolution of the industry is the adoption of EMR's. In many cases, solution design happens without communicating with end user. Often, end user himself/herself doesn't want to get involved in the process. In such case, decision maker in the hospital (or health system) want to rely on the expertise of the solution provider and that's why they often contract only 'big name' firms. That situation is normal looking into the fact that this industry started only during the last decade in middle east. However, that leads to a situation where the user feels that the solution is enforced and; thus, lot's of resistance will be there to adopt the new solution. In fact, in many cases, user doesn't feel the ownership of this solution and I have seen some cases where users literally thrown away patient data to litter bins as part of a decision to change the solution provider. The solution to this odd situations will naturally come when when local management in GCC gains more expertise in both: field of Healthcare Technology and the field of Supply chain management and when the No. of Health IT experts grow up. Having such local expertise will shift trust from third parties to their own in-house experts. Those experts will naturally start getting more involved in the process of the solution design and thus will pretty much help in the change management and the adoption of the new systems.
Unique Situation.
On the other side, there are examples of some problems that are unique in middle east and the legal, political, financial systems in GCC play a big role in them. In fact, due to booming oil prices, GCC states have been allocating a good amount of their money for the development of their healthcare systems. Much of it goes to building new medical facilities and state of art medical technology. This is unique because almost every where in the world, economy was in hiccup except in emerging markets. GCC is still unique due to the high liquidity and due to the high GDP per capita. That will make buyer in GCC very hungry for consumption. Running a supply management system for a hungry market is not an easy task. In fact, majority of health systems in GCC are state owned which makes the procurement process very slow compared to privately owned facilities. So, high spending in a slow process, means that spending will only be in big-chunks. In such areas, supply chain management is not easy. If you push to speed up, you inject more money to corrupted practices, if you tighten the spending's; then you are slowing down the development process.
That shows up very clearly in EMR operation when; for example, EMR needs to be upgraded to a new version. That's is sometimes critical for the operations as new versions usually include fixes for previous bugs, or new feature that are necessary for the operation. In such cases, supply chain systems will not be able to finance those upgrades to new version immediately as the procurement process take long time. Thus, EMR management usually runs in trouble not because of shortage of money but rather running short in money on the right time. The solution to this problem lays; from my point of view, in the adoption of modern supply chain management techniques that will balance between spending and needs, by committing to evidence based purchasing and by decentralizing the supply management. In that case, the role of central management should be constrained to guidance, and motoring rather than direct control.
My final advice to GCC is to focus on the quality of spending on health technology rather than to focus on the amount of spending. Once we focus on quality this will automatically lead to an in depth perception and thus a sustainable solution to our problems in healthcare at GCC.

إدارة موجودات نظم المعلومات في مستشفيات الكويت .. الآية المقلوبة


" الآية مقلوبة " ..
هي عبارة نطلقها في معرض تعليقنا على ظاهرة معينة ليست في محلها خصوصا تلك الظواهر التي تكون على النقيض تماما من الوضع الطبيعي.

الرسم البياني أعلاه هو لإحدى الإسئلة التي طرحت في استفتاء نشرت نتائجه في المؤتمر الخليجي للطب النووي 2013.  السؤال يقول "من هو المسئول عن جرد ممتلكات القسم الطبي من أجهزة وبرمجيات خاصة بنظم المعلومات؟ ". فكانت النتيجة كما ترون في الشكل البياني اعلاه.

الإستبيان
4% أجابوا بأن لايوجد هناك أي نوع من الجرد لأجهزة وبرمجيات نظم المعلومات في القسم الطبي.
13% أجابوا بأن المسئول هو قسم نظم المعلومات (رغم أن لاوجود لقسم المعلومات. على ارض الواقع في مستشفيات وزارة الصحة )
30% أجابوا أن المسئول هو مدير شبكة الباكس (إختصاصي المعلومات في قسم التصوير الطبي)
52% قالوا بان مزود النظام (او مزود الخدمة) هو المسئول عن الجرد!

اذاً وبدون لف ودوران ، عندما يعتقد اكثر من نصف المستفتين بان عمليات ادارة الموجودات (Inventory System) وجردها ومراقبة حركتها وصيانتها هي مسئولية الشركة مزودة الخدمة !!
لذا فإنني اقولها بالفعل بان الاية مقلوبة في بعض مستشفياتنا. اذ ان هذه العمليات هي احدى العمليات التي عادة لاتوكل الى جهة خارجية ثالثة لارتباطها الوثيق بالنظام الاداري والمحسابي الدقيق والمحدد.

المشكلة
إيكال مهمة ادارة موجودات نظم المعلومات في اقسام التصوير الطبي على سبيل المثال للشركات المزودة لنظام الباكس هو امر غير سليم. و أثارهذه المشكلة هي كالتالي:


  1. الاجهزة والبرمجيات المطلوب حصرها هي من ممتلكات القسم الطبي والمستشفى فلماذا يقوم مزود الخدمة باجراء هذه الخدمة على اكمل وجه ، وبدون مقابل ، وبامانة وصدق؟! 
  2. هذه المهمة ليست من الصلاحيات المتفق عليها في عقد توفير الخدمة ، ولذلك فإن هذا سيكون مدعاة للفوضى و منطقة رمادية مثيرة للمشاكل بصورة مستمرة.
  3. على أرض الواقع إدارة الموجودات في القسم الطبي يديرها القسم نفسه و يوثقها قسم ورشة الهندسة الطبية أو قسم نظم المعلومات خصوصا لأجهزة الكمبيوتر التي وفرها قسم نظم المعلومات من البداية. وعدم وضوح المسئولية وتشارك قسمان مختلفان فيها هو سبب للربكة وسوء الإدارة.

جذور المشكلة
للمشكلة سببان:
  1. لازالت البنية التحتية لنظم المعلومات في أغلب الأقسام الطبية تزود من قبل مزود نظام المعلومات الطبية (كمبيوترات - شبكة - طابعات - سكانرات - سيرفرات - برامج حماية - دومين كونترول) ، ولازالت عقود صيانة هذه النظم ومراقبة عملياتها تتبع قسم الهندسة الطبية.  وللأسف فإن إدارة الهندسة الطبية في المستشفى ليست ذات خبرة كافية في التعامل مع أنظمة المعلومات وإدارتها.
  2. ولنفس السبب السابق فإن هناك خللا في نظام الجرد لنظم المعلومات. حيث إن إدارة الهندسة الطبية لاتجرد كل جهاز بل تتعامل مع جميع أجزاء نظام المعلومات على أنه جزء واحد (تقريبا) وتحت عقد واحد (تقريبا) ، وعليه بإمكانكم تخيل المشكلة. فكل جهاز كمبيوتر يتعطل أو طابعة أو جزء من الشبكة فإن ذلك قد يعد عطلا للنظام بأكمله (حسب الأوراق الرسمية) بالإضافة إلى أن التعامل مع النظام ككل كجهاز واحد مسجل تحت رقم جرد واحد فإن ذلك يعني أن المستشفى مجبر على التعاقد مع مزود خدمة واحد.
  3. في الحقيقة كانت هناك محاولات لايكال مهمة جرد وإدارة موجودات نظم المعلومات لإدارة المعلومات في بعض المستشفيات ، ولكن هذه المهمة فشلت بسبب رجوع ملكية هذه الموجودات لجهة تختلف عن الجهة التي تدير هذه الموجودات.

الحلول
  1. يجب أن تنقل ملكية نظم المعلومات وأجهزتها إلى قسم نظم المعلومات في المستشفى سيما بنية المعلومات التحتية كأجزاء الشبكة الإلكترونية ، الدومين كونترول ، الكمبيوترات ، الطابعات ، وأجهزة التخزين ، وتبقى ملكية أجهزة وبرامج التشخيص وشاشات العرض الطبية تحت ملكية القسم الطبي قسم التجهيزات الطبية.
  2. كل جهاز في عهدة المستشفى يجب تسجيله في نظام الجرد وكشوفها حتى الطابعات وأجهزة الكمبيوتر لتلافي الضياع والتلف والتلاعب وإدارة الصيانة.
  3. يجب أن توكل عقود الصيانة والدعم الفني كل حسب إختصاصه. فعلى سبيل المثال: عقد إدارة ومتابعة وصيانة الشبكات توكل لشركات متخصصة في الشبكات وليس بالضرورة لشركات الأجهزة الطبية لأنها شركات مختصة وتمتلك خبرة أكبر في التعامل مع نظم المعلومات.
  4. إنشاء مركز اتصال لأعطال أنظمة المعلومات (HelpDesk/Call Center) تابع للمستشفى ككل وهو المسئول عن تغطية الدعم الفني لأي جهاز في نظام المعلومات ، وهو المسئول عن توزيع أوامر التصليح وتحويلها إلى الجهة المختصة والشركة المقدمة لخدمة الدعم الفني ويجب أن يعمل هذا المركز على مدار الساعة.
  5. الحلول أعلاه سوف تفرغ اختصاصصي المعلومات الصحية ومنهم مدير شبكة الباكس لمهام عمله الأصلية وهي إدارة المعلومات الصحية وإدارة جودتها والتدريب وإدارة مشاريع التطوير والدعم الفني للترابط بين "الأجهزة الطبية".
  6. تفعيل نظم معلومات إدارة الموجودات (Inventory Management Systems) والتي قد توجد في نظام واحد مع نظام معلومات الأعطال ومنها على سبيل المثال نظام شركة (SysAid Technologies) الذي يحتوي على أجزاء عديد لإدارة نظم المعلومات ومنها: إدارة الموجودات - نظام تلقي الأعطال - نظام قاعدة بيانات الأعطال - نظام إدارة جودة قسم المعلومات - إلخ..

المعايير العالمية للمعلوماتية الصحية .. لمن يهمه الأمر!!



  1. المعلوماتية الصحية تحتاج إلى قبول من المستخدمولا ينبغي أن تكون مفروضة .. نعم ينبغي أن تفعل بقرارات جادة وحاسمة ولكن بتخطيط وتدرج.
  2. نظم المعلومات الصحية ليست صناديق سوداء
    بل نظم مبنية على معايير عالمية و لكن محضرة في مختبرات الشركات وأغلبها يعتمد في النهاية على تكنولوجيا قياسية.
  3. نظم المعلومات الصحية قابلة لأن تتبادل المعلومات فيما بينهانحتاج فقط أن تتأكد من التزامها بالمعايير العالمية اللازمة لترابط المعلومات.
  4. معايير عالمية لنجاح مشروع الملف الصحي الإلكتروني في الكويت
    المعايير العالمية للمعلوماتية الصحية وتطبيقها هما الضمان للنجاح.
  5. تطبيق المعايير العالمية في المعلوماتية الصحية يعطي الحريةو يمنع من الأحتكار .. فكر ما ذا لوقررت الدولة إلغاء عقد مزود الخدمة ؟!
  6. المعايير العالمية للمعلوماتية الصحية هي الضامن لسهولة التطوير
     والتوسع في عالم نظم المعلومات المتغير بشكل سريع.
  7. نظم المعلومات الصحية (لا يجب) أن تكون من مزود خدمة واحد
    بل يمكن أن تكون من مصانع مختلفة و يكفي أن تجرى لها اختبارات الترابط قبل وبعد الشراء.
  8. الإستعجال في مشاريع المعلوماتية الصحية الكبرى سبب للفشل
    دون الرؤية الواضحة للأهداف نصل إلى الفشل و الفشل إلى المزيد من الربكة في الخدمات الصحية.
  9. مشاريع المعلوماتية الصحية بجب أن تبدأ صغيرة وتنتهي كبيرة
     يفضل تقسيم المشاريع الكبيرة إلى مشاريع صغيرة جغرافيا وإداريا مع ضمان الترابط حسب المعايير العالمية.
  10. المشاريع الضخمة عودة إلى المركزية
    لأنها تعني فرق عمل ضخمة يصعب بينها لإتصال وغالبا غير متجانسة ولذلك فشلت مشاريع الصحة الإلكترونية في كثير من الدول المتقدمة كالمملكة المتحدة.

Advanced Visualization in Kuwait


نظام التصوير ثلاثي الأبعاد في قسم الأشعة بمستشفى الجهراء


يستخدم في تشخيص الكثير من الأمراض مثل ضمور الدماغ (كما في الصورة)، وأمراض أخرى كانسداد الشرايين وتكلس الشرايين وغيرها .. وتتميز أنظمة التصوير الثلاثي الأبعداد (Advanced Visualization) بأنها تسهل تصوير فسيولوجية وتشريح أعضاء الجسم لتسهيل تشخيص الأمراض دون الحاجة إلى خبرة عالية في تشخيص الأمراض عن طريق الصور ثنائية الأبعاد والتي تتطلب خيالا وخبرة عاليتين. و قد توفر هذه الأنظمة صورا متحركة تستخدم في تشخيص أمراض لايمكن تشخيصها بسهولة كالفحص التخيلي للمنظار القولوني (Virtual Colonoscopy).

يعرف عن هذه الأنظمة أنها عادة ما تحتاج إلى مهارة في تجهيز الصور ليقوم الكمبيوتر بإعادة تركيبها ومعالجتها فتظهر في صورة جديدة سواء ثلاثية الأبعاد ، أو على شكل صور متحركة ، أو على شكل تقرير رقمي تشخص فيه فيسيولوجية القلب. لذلك فإن التدريب المستمر على هذه الأجهزة هو من أهم الأشياء التي ينبغي على المستشفيات توفيرها ووضعها في حساب التكلفة السنوية لتشغيلها.

تمتلك الكويت أكثر من 45 جهاز من هذه الأجهزة (35 جهاز في العام 2007) منها حوالي 9 في قسم الأشعة بمستشفى مبارك الكبير ، صرفت الكويت عليها حوالي 1,072,000 د.ك منذ العام 1996 ، وحوالي 135,820 د.ك سنويا على أعمال الدعم الفني والصيانة لهذه الأنظمة.

وهذه الأنظمة ورغم أهميتها في التشخيص الطبي في يومنا هذا إلا أنها لاتستغل الإستغلال الأمثل إذا ماقورنت بالمبالغ المهولة التي تصرفها وزارة الصحة الكويتية عليها.

When viruses attack ICU

Alerts at ICU

Intensive Care Units (ICU) are places in hospitals where people with special care are placed. Those people need continuous monitoring for there condition. These units are normally equipped with high tech patient monitor machines, ECGs, and ventilators.

Furthermore, ICU's normally keep higher standard of infection control to make them invulnerable to all kinds of infections due to the critical condition of it's patients.

Last, week I was in Al-Amiri hospital, Kuwait. I was doing maintenance to a portable xray machine. There, I was struck with a view on all PC screens. Those PC's were running Critical Care Information system. In fact, all screens where showing a Mcfee Virus Alert. The list; as you see above, is showing multiple virus infections.

Even though ICU was equipped with high-tech patient monitors, ventilators and looked very clean and dis-infected, but it was; actually, infected with a special infections. These infection have no direct harm to the patient but they eventually do a harm !

How does a computer virus threatens life?

In these days, patient monitoring is not done at each bed. Each patient is monitored centrally by nurses & medical staff; i.e., at the nurse station. Thus, interruption in flow of clinical data between patient monitors and central monitoring system will definitely interrupt monitoring the vital signs of critically ill patient's .

Second; at critical care stations, physicians make electronic requests; i.e., for lab or imaging procedures. Virus infections can make those PC's slow or make them completely down by corrupting some files. In fact, delay in exam requests is definitely a life threatening .

Third, at critical care stations, physicians write there reports, notes or prescribe patient's medication. Virus threats on those stations; again, will delay the whole process at the ICU. Delay is not a good sign of a good ICU.

On the ground..

Frankly speaking, this ICU was not the only place in Ministry of Health where I have seen such virus attacks. Many places like neonatal ICUs, Radiology Departments, Cardiology, and Nuclear Medicine had the same issue.

In 2008, one year after world-wide attacks of a well know electronic warm(special viruses). the warm(or Trojan) with different names such as (DownaDop, Conficker, etc...) have hit most of ministry's information systems in a mater of a week. Moreover, xray and ultrasound machines were also hit by this warm specially those with MS-Windows XP were badly hit. Around 6 Xray machines went totally down due to network problems.

Finally, we have to admit that hospitals in Kuwait are in trouble ! In near Future we might see a crisis due to the fact that the no. of medical equipment getting connected using TCP/IP networks is exponentially growing.

Problem and Solution !

The problem is not actually the "virus attacks", virus attacks happen every day and everywhere, the problem is lack of network security , and maintaining that ! In fact, If you are connected to the outside world then you are at risk ! you don't have to be connected to the internet, allowing use of removable storage devices (i.e. USB sticks) is the biggest source of invulnerability.

The solution is not easy but affordable and better to start now than never.Effort to set standards for security in eHealth and for medical devices is going on. HIPAA is one example of such standards. HIPAA is a governmental act in USA to assure security and privacy of patient's health records. Some of these requirements are securing health records from viruses.

Moreover, and effort was taken by U.S department of veteran affairs to design a model for secure network of medical devices. See Excellent readings. Another effort was taken by Society of Hospital Information Mangament Systems (HiMSS) to encourage a medical device security disclosure like DICOM conformance statement. See Excellent Reading below.

In Kuwait, the gap is very huge. We should immediatly start some where. Niether we have IT team in hospital nor Biomedical Engineering departments have experience or a say in this.

Thus a very rough plan should be divided into three stages.

  • Stage 1. Putting IT security standards on our list of priorities.

  • Stage 2. Forming an independent IT support team in the hospital independent from medical device vendor. If MOH claims that they don't have enough resources, IT team doesn't have to be through local staff, IT can be outsourced just like electrical maintenance or waste Management .

  • Stage 3. To Include IT support, Network management , and network security in medical device RFP, and the service contract (service level agreement.)

Mubarak Hospital (~800 bed hospital) is undergoing it’s third generation PACS. In the third time, radiology department realized that they cannot depend on vendor in setting their PACS/RIS requirements since they suffered a lot win their previous PACS Archive got full.

They decided to help the vendor by putting their requirement in the form of an RFP. Even though this RFP contained fatal mistakes, but it was certainly better than leaving every thing up to the vendor.


Their RFP writing process included PACS Storage calculation with help of their PACS Administrator Mr. Sadiq bin Hussein and Mrs. Abeer Hajiya.

After Long time, announcement of the winner of last month’s online contest is here. She is Dr. Salwa M. Hayder who is known for her care to patient and to her career.   • How do you find the contest in general ?
        
    Dr. Salwa:  Very interesting and challenging.

  • How hard was the contest questions ?
        Dr. Salwa:  I believe it was hard especially it asked for some details which is out of my specialty, mainly  medical engineer will know it.

  • Did you do little research to answer the contest questions ?
       Dr. Salwa:   Yes

  • Do you think that details about HL7 messages, Diagnostic Monitors or any other PACS/RIS option should be added to the Tender document (RFP) ? why ?
      Dr. Salwa:   I think its better to cover all the details in the tender to make sure the system will work smooth with other hospital systems.

  • Do hospitals in Kuwait verify if these requested options are delivered with PACS/RIS ?  Dr. Salwa:   I know that they mainly insist on having HL7 compatibility with no more details.          Thank you

    Q1What is the “eSec Login” option is ?
    A1. “eSec” is abbreviation of eSecurity.. which is a security option which allows radiologists to login to Centricity RIS using a magnetic card, which obviously requires a card reader Hardware.
